M. Batshuayi has scored a goal for Chelsea! Assist by Cesc Fàbregas.
Fabregas played the ball in behind for Batshuayi, who took his time to amble after the pass, just onside, and score.
GOAL! A great end to a great day for Chelsea! Batshuayi cuts inside from the right to fire a fine goal with his left foot on the final whistle. 5-1.
90' +1
There are just the two minutes to be added here.
90'
M. Batshuayi has scored a goal for Chelsea! Assist by Pedro.
Pedro fed the ball through for Batshuayi, Pickford's kick having fallen hopelessly short at the winger's feet.
GOAL! That's number four for Chelsea! Batshuayi scores for a third consecutive game as he slides a finish under Pickford. 4-1.
89'
CLOSE! Azpilicueta, one of the centre backs, has been stationed on the edge of the Sunderland box for some time. He sidefoots just wide of the right-hand post.
88'
D. Gibson enters the game and replaces L. Cattermole.
SUB! Cattermole off for Darron Gibson for Sunderland.
The game has now been reduced to a walking pace, but Willian still has the energy to work room for a shot on his left. He fires over.
85'
"Antonio, Antonio.." sing the home fans. Diego Costa is chatting happily on the bench as he, like the majority of those in attendance, gets ready to celebrate.
84'
Again Batshuayi is denied! He takes Pedro's square pass, but there's a three-man red and white wall stood in his way in the area.
82'
Moses shoots, blocked, Batshuayi pounces on the ball but there's no way through for him either. Sunderland are desperately trying to keep the scoreline somewhat respectable.
81'
Still, Sunderland keep running hard. They've simply been outclassed today by the champions.
Minute
Description
79'
There's a slight stoppage after the goal, Pickford seemingly having injured himself as he desperately clawed at Pedro's header.
77'
Pedro has scored a goal for Chelsea!
GOAL! Oh dear, Sunderland. Fabregas spots Pedro's run in behind and knocks a pass long, but both Pickford and Lescott come for the ball and the latter nods beyond the former. Pedro is on hand to head into the net. 3-1.
76'
Moses cannot quite force a cross in from the right under pressure from Lescott, with Chelsea now turning to an array of fancy flicks. In the stands, the fans serenade club legends with a series of songs.
74'
High and wide from Pedro! Taking a pass from Willian, he couldn't whip a strike into the top-right corner.
73'
Fabregas switches from deep towards Alonso, Manquillo heads behind. Corner. Sunderland are hanging on right now.
71'
Pedro enters the game and replaces E. Hazard.
SUB! With the cup final in mind, Hazard makes way for Pedro.
70'
CHANCE! A tough opportunity for Batshuayi in the end, fed by Hazard on the right, he was forced wide before shooting into the sidenetting.
69'
As with just about every other set-piece so far today, Sunderland cannot clear. Moses finally delivers and loops a high ball straight into Pickford's hands.
68'
Wonderful by Moses and so close to a Sunderland own goal! The Chelsea wing-back twists this way and that, crossing powerfully for Rodwell to hack behind for a corner, just missing the net.
67'
Hazard flashes a drive across the face of goal, but Batshuayi is again caught on his heels. Chelsea keep pushing.
66'
And almost a chance for Batshuayi! Willian looked to slide him in for an open goal, but the substitute didn't read the intention.
65'
A marginal offside call halts a Sunderland attack as the noise levels ramp up again. Chelsea are well on their way to a 30th league win of the season in front of a raucous capacity crowd.
62'
L. Gooch enters the game and replaces A. Januzaj.
M. Batshuayi enters the game and replaces Diego Costa.
SUB! And Diego Costa waves to the crowd as he is replaced by Michy Batshuayi. He's been linked with a move away, but Conte will surely be keen to keep his top-scorer.
SUB! A Sunderland change, too: Lynden Gooch on for Januzaj.
61'
E. Hazard has scored a goal for Chelsea! Assist by Diego Costa.
GOAL! Chelsea have the lead and it's Hazard! He collects the ball on the left and batters a fine strike into the far corner. Superb. 2-1.
Willian fed Diego Costa, who turned to play in Hazard, providing an assist just seconds before being withdrawn.
59'
A couple of hopeful penalty appeals from the hosts. Cesar Azpilicueta slips, his cross just clips Lescott's arm and then Diego Costa goes to ground. Any award would have been very harsh.
57'
Sunderland again struggle to clear! Lescott nods straight to Moses, who skips into space and drills across goal, David Luiz just failing to make contact at the back post.
56'
A Chelsea corner. Moses struggles to find space but crosses, a deflection and then Pickford's clumsy handling turning the ball behind.
55'
A crucial challenge from N'Golo Kante! He's rarely been troubled so far, but Fabio Borini's forward drive forces the Chelsea midfielder into a superb recovering tackle.
54'
Moses does now make progress at the other end, but wildly lashes a strike out for a throw-in to jeers from the away end.
53'
Chelsea are yet to really get going in the second period, escaping a slightly nervy moment as three red and white shirts close down Courtois.
51'
SAVE! Sunderland win a corner on the right and Rodwell's header from the delivery sends Courtois scrambling. Chelsea just about clear.
50'
After a muted spell following the break, the noise levels are rising again in the stands, fans on their feet to chants of "Stand up for the champions".
48'
B. Jones gets yellow.
YELLOW! Sunderland had made a good start to the half, penning Chelsea in, but Hazard got out and was swiped at hopelessly by Jones. The Sunderland man goes into the book.
46'
KICK-OFF: Diego Costa gets the game started again.
45' +2
HALF-TIME: Chelsea 1-1 Sunderland.
45' +1
There's just one minute of stoppage time to be played, despite the lengthy pause as Terry was substituted.
44'
SAVE! O'Shea has two bites at the cherry from the set-piece, Cahill only clearing as far as the Sunderland captain, but Courtois comfortably gathers.
As per, Chelsea race up the other end. Hazard finally shoots from the left, but he cannot keep his strike down.
43'
Partially aided by the stoppage for Terry's withdrawal, Sunderland have slowed Chelsea's momentum now. They have again earned a free-kick in attack, too.
41'
He's a man on a mission now, though, Diego Costa. Some neat build-up play concludes with Hazard flicking towards the forward, who ignores his team-mates to shoot wide.
38'
Diego Costa gets yellow.
YELLOW! And now Diego Costa is frustrated as he and O'Shea clash on halfway, each waving their arms towards the other. The Chelsea man goes down, but O'Shea punished only by a free-kick as Diego Costa is booked.
36'
While the rest of the Chelsea cast enjoy themselves, Diego Costa has looked a little sluggish. He forces a corner, but only after miscontrolling as he looked to shoot. Pickford takes the set-piece easily.
34'
Willian crosses, Diego Costa stretches, but Jones shields the ball and Pickford gathers at the second attempt.
33'
Again Sunderland struggle to clear, but Hazard cannot force his way through and Pickford and his back-line can breathe again.
32'
CLOSE! Willian has been heavily involved, too, and almost grabs a second. Moses feeds the Brazilian on the left and, cutting inside, his shot is deflected past the post by Billy Jones.
31'
Fabregas and Hazard continue to show off their immense skills as they swap passes on the edge of the box. The former centres, but Pickford holds on.
30'
CLOSE! A long Chelsea move ends with the ball at Willian's feet, the winger flashing a strike across the face of goal.
28'
G. Cahill enters the game and replaces J. Terry.
SUB! Terry takes his leave! Pickford sportingly kicks the ball out in minute 26 - the same as the Chelsea captain's squad number - and Terry is quickly surrounded by team-mates offering their best wishes. He is given his own guard of honour and, clearly emotional, passes the armband to replacement Gary Cahill.
25'
And again Sunderland cross from the right. This clearance is poor, allowing Bryan Oviedo to keep the attack alive. The ball breaks for Cattermole, whose effort is even worse this time and drags wide.
24'
An energetic Rodwell burst gives Sunderland further respite, but, as he reaches the right-hand corner flag, his cross is nodded away.
23'
Januzaj holds the ball up on the left for Sunderland, finally allowing them to enjoy some possession. Lee Cattermole takes the pass, though, and lashes over from distance.
21'
Still Sunderland struggle to clear. The ball comes straight back and Hazard tees up David Luiz. Yet another deflection, yet another corner. David Luiz meets this cross, too, but nods well wide.
20'
CHANCE! Fabregas' corners are twice cleared at the near post, before Alonso gets in to cross. Diego Costa cannot get a shot off, but Moses can and sees his effort deflected over the top.
19'
This is lovely stuff from Chelsea. Hazard and Fabregas trade passes on the left and suddenly Terry appears on the overlap. He wins a corner.
18'
Larsson's delivery is deep, towards the far post and Courtois catches. Chelsea are quickly back on the attack.
17'
Despite the early Sunderland goal, Chelsea look in the mood today. The visitors have won another free-kick at the other end, though, with fouls aplenty in the opening exchances.
16'
CHANCE! Another opening! David Luiz resists the temptation to shoot and picks out Diego Costa, who volleys just wide.
14'
Another Chelsea free-kick as Rodwell again fouls Hazard. Sunderland will hope to defend this set-piece rather better than the last.
12'
CHANCE! Chelsea should be in front! Willian slips in Moses, who crosses towards Diego Costa in the centre. He fails to make contact and Hazard, with the goal gaping, cannot turn and prod the ball into the net.
SAVE! This could be a long 90 minutes for Pickford and Sunderland. Chelsea again line up to shoot and Alonso thunders a strike towards goal, the keeper saving well.
10'
Sunderland had 10 men in the box as Chelsea netted and that appears to be their gameplan to stifle the champions, with a wall of red and white shirts again forming in front of the hosts from the kick-off.
8'
Willian has scored a goal for Chelsea!
CROSSBAR! Oh so close to an equaliser! Marcos Alonso, formerly a Sunderland loan signing, curls a superb strike against the angle of crossbar and post from the set-piece.
GOAL! Now Chelsea are level! Willian nets as Sunderland fail to clear from the free-kick, a number of men in blue attempting to force a shot on target before the ball fell for the Brazil international. He struck hard and low and through Jordan Pickford. 1-1.
7'
Now Chelsea have a free-kick, though. Jack Rodwell upends Eden Hazard in a similar position to where Adnan Januzaj was fouled at the other end.
4'
"One-nil to the Championship," sing the visiting supporters.
3'
Javier Manquillo has scored a goal for Sunderland!
GOAL! Javier Manquillo scores for Sunderland! The champions are stunned as Larsson's free-kick is blocked by the wall but deflects away from Victor Moses, arriving at Manquillo's feet. He thrashes home and Sunderland lead. 0-1.
2'
And the visitors have started brightly, winning a free-kick around 25 yards from goal, with Sebastian Larsson lining up to take...

KICK-OFF: Sunderland get us under way!
Hello and welcome to our live text commentary of the last Premier League match of the season between champions Chelsea and relegated Sunderland from Stamford Bridge!
Chelsea will today lift the Premier League trophy for a fifth time, also matching Sunderland's overall tally of six top-flight titles after vastly contrasting campaigns for the two clubs. Results elsewhere cannot impact either side's fortunes; David Moyes' men will finish bottom of the table.
And on that note, let's get to the team news...
However, the visiting players are now fighting for their futures at the Stadium of Light - or possibly eyeing a move away - while the Blues will hope to impress Antonio Conte ahead of next weekend's FA Cup final against Arsenal.
Chelsea: Courtois, Azpilicueta, David Luiz, Terry, Moses, Kante, Fabregas, Alonso, Willian, Diego Costa, Hazard.
Moyes matches Chelsea's back three as Joleon Lescott is handed a first start for Sunderland. Jermain Defoe, hotly tipped for an off-season move, misses out through injury.
After making a number of changes for Monday's match against Watford, Conte recalls his star men, including top-scorer Diego Costa. John Terry, set to leave the club in the coming transfer window, leads Chelsea out on home turf one last time.
A Cesc Fabregas goal was decisive as Chelsea won the reverse clash 1-0 in December, while the Blues were winners in this fixture on the final day of the 2014-15 season, again topping the table. Sunderland have won on two of their last six visits to the Bridge, though, despite a decade largely made up of struggle in the top tier.
Victory today would make Chelsea the first Premier League side to record 30 wins in a 38-game campaign. Number 29 was a thriller - 4-3 against Watford - and the home crowd will hope for a similarly entertaining contest as the curtain falls on a successful season.
"We have two targets: to try to win the 30 games in this league and to help Thibaut [Courtois] to win the Golden Glove," Conte said this week. Courtois has kept 16 clean sheets in the league this season - one more than Tottenham's Hugo Lloris at the top of the standings.
Diego Costa's hopes of taking the Golden Boot are surely over, however, with Harry Kane now six goals clear of the Chelsea front man.
The Sunderland side are in the tunnel, ready to head out onto the pitch and provide Chelsea with a guard of honour. The stands are bedecked with blue and white banners as the home fans get ready to party.
John O'Shea leads out the visitors and, now, here come Terry and Chelsea! Kick-off is just a couple of minutes away.
Manquillo scored early for Sunderland, but Willian quickly levelled as Chelsea dominated long stretches of the half. Perhaps the most notable moment, though, came when Terry bade farewell to the Bridge with a 26th-minute substitution.
Terry bowed out with a fitting tribute, too, while Courtois takes the Golden Glove courtesy of Hull City scoring against Tottenham elsewhere.
Chelsea have win number 30 - and in some style. Goals from Willian, Hazard, Pedro and Batshuayi, twice, made light work of Sunderland, who had scored through Manquillo.
A trophy presentation and further farewells from Terry will follow at the Bridge, but that's all from this Premier League season. Brilliant, brilliant Chelsea have ended a glorious campaign on a high.
Thanks for joining us this season and, until next time, goodbye!
