Ayew makes the slow walk to the touchline as he's replaced here, winding precious time off the clock ahead of a Baggies free-kick, with Stephen Kingsley entering the fray.
90' +2
F. Fernández gets yellow.
Fernandez goes into the book as West Brom mount a push here for the equaliser.
90' +1
J. Leko enters the game and replaces M. Wilson.
Nordfeldt on hand to deny Evans! Great piece of goalkeeping from the Swede, who earns his keep late on here, atoning for his earlier error to this time deny the West Brom centre-half from heading home from a set-piece!
Jonathan Leko replaces Wilson with West Brom's final change of the day.
90'
There will be a minimum of three minutes added on at the end of this match. Is there to be another twist in the tale?
88'
Swansea are all over West Brom in the dying stages here, full of pep and zest following this impressive comeback! There's a great enthusiasm around the Liberty Stadium, on what's shaping up to be one heck of a party in Wales tonight! In fact, they've been partying all week since securing their top-flight status a week ago!
86'
Fernando Llorente has scored a goal for Swansea City! Assist by L. Narsingh.
FROM BEHIND, SWANSEA LEAD! Unbridled joy around the Liberty Stadium as they cap off their Premier League survival with a spirited showing in the second half today! Llorente has had a taste for goals in this push for survival, and he adds another for good measure, beating Foster with a composed first-time finish on the end of a delightful cross from Narsingh!
Narsingh picked that out expertly, as he comes off the bench to assist what stands as the winning goal for the time being!
84'
H. Robson-Kanu enters the game and replaces J. Morrison.
Hal Robson-Kanu replaces Morrison as the Baggies make their second change.
83'
STAT: Gylfi Sigurdsson needs one more goal to record double figures for both goals and assists this season. He would be the first Swansea player to achieve this in a Premier League season (nine goals, 13 assists).
81'
Ki Sung-Yueng enters the game and replaces L. Fer.
Rondon continues to hold the ball up brilliantly, providing the determined bit of play the Baggies needed here, winning a free-kick as he does very well to fend off Carroll. The ensuing delivery comes in for Dawson at the far post, but the centre-half is penalised for a foul.
Swansea make their second change of the afternoon here as Ki Sung-Yeung replaces Fer.

Description
79'
J. McClean enters the game and replaces J. Livermore.
James McClean is brought on as West Brom go to the bench for the first time this afternoon, with Livermore the one replaced.
78'
The tables have turned in a big way here, as West Brom are struggling to get a touch on the ball now, after looking to be in complete control just 10 minutes ago. It's on a knife's edge now, as we creep towards the last 10 minutes in the 2016/17 Premier League season!
76'
There's a different feel to Swansea now as they push on for the winner, and very much a driving force behind Ayew now, who has a taste for goals. He pieces together a terrific run and smacks a shot at goal, but it winds up crashing into the side-netting!
74'
STAT: Paul Clement has won eight of his 18 Premier League games this season as Swansea City manager, more than double the number than Francesco Guidolin, Bob Bradley and Alan Curtis won in their 18 games combined this season in the competition (3).
72'
J. Ayew has scored a goal for Swansea City! Assist by Fernando Llorente.
Lovely set-up by Llorente who unselfishly squared for Ayew to pick up the assist, but Sigurdsson played a prominent role in that as well.
LEVEL AT THE LIBERTY! Sigurdsson the architect with yet another delicious free-kick, placed on the head of Llorente who guides it into the centre for Ayew, and the Ghanaian pounces to nab his first goal of the season on this, the final day!
70'
Great claim from Foster, who has been ever-present for the Baggies this season, playing every minute of Premier League action for them - and he's capping his campaign off with a strong showing today. He hasn't been tested often, but when he has he's been sharp, navigating through a sea of bodies to punch Sigurdsson's free-kick clear on this occasion.
68'
Rondon continues to prove a nuisance for this Swansea defence, who are lacking in terms of support now with Britton off. The hosts needed to make the change, with the hope that Narsingh can help provide some refreshingly direct play going forward, which they have sorely been lacking.
66'
L. Narsingh enters the game and replaces L. Britton.
A first change here for Swansea as Luciano Narsingh replaces Britton, who has been so crucial in ushering the Swans to safety - and he's showered with applause as a result as his season comes to an end.
64'
STAT: Swansea have won four of their five home Premier League encounters with West Brom (L1), hitting three past the Baggies on three occasions.
62'
Penalty appeals ignored! Olsson looked to have Dawson beaten for pace as he gets on the end of a clever throughball from Llorente, before going down in a heap. The timing of the drop from Olsson was poor, however, as there wasn't enough contact to justify his going to ground.
60'
Livermore takes on Mawson in the area, after a friendly ricochet to create that chance for the visitors. The midfielder does well to switch onto his right and open up room for a shot, but the angle is tight and the finish is lacking in the end as he fails to hit the target.
58'
Swansea have not done much to test this defence. Sigurdsson has dropped deeper to try and provide a link between midfield and attack, but it's left Llorente very much isolated up top as a result. It may be time for Clement to make a change.
56'
West Brom are in complete control at the moment following the slight scare following the 50-minute mark. Swansea have been dominated in midfield today and they've been unable to remove the shackles placed on them by the Baggies.
54'
STAT: Fernando Llorente has scored nine goals in his last 10 Premier League appearances for Swansea City at the Liberty Stadium.
52'
Great delivery from Sigurdsson in towards the far post and Llorente attacks it, but just can't get much contact on the delivery. It lands right in the danger area, but West Brom do enough to put it behind. The ensuing delivery comes right in for an unmarked Fernandez, but he sends his header wide from point-blank range! That was a sitter!
51'
M. Wilson gets yellow.
Wilson becomes the first player booked in the match as he clips the heels of Ayew on the edge of the box, conceding a free-kick in a very dangerous position for Sigurdsson to whip in a cross.
50'
Has to be better! Great work from Rondon to overpower Fernandez, with the aid of a possible foul that went uncalled from referee Mike Dean, as the striker gets himself through on goal, but can't finish the move as Britton does brilliantly to slide in from the back and put in the timeliest of blocks!
48'
Bright start to this second half in Wales! Brunt tries to catch out Nordfeldt from a free-kick from range, and nearly does, before Swansea break the other way with Sigurdsson sending in Llorente, but the striker lacks the pace to stay behind the defence.
46'
Back underway at the Liberty Stadium!
45' +1
HALF-TIME: SWANSEA 0-1 WEST BROM.
45'
There will be one minute added on at the end of this first half.
43'
Perhaps this bit of feistiness will ignite a fire under Swansea, who are in need of a bit of inspiration in this match. Their complacency can be forgiven, considering how hard they have worked to get to this point, but they would still like to end the season with a result.
41'
Into the final five minutes of the first half and it continues to bubble over as it's close to reaching a boiling point. Olsson and Nyom at the heart of hit again, this time with the Swansea full-back on the receiving end of a foul. Now Pulis is involved, running over to shout at Olsson from the touchline!
39'
STAT: Tony Pulis has yet to record a top-flight victory at Liberty Stadium (D1 L3), with his sides scoring just twice in four games.
37'
Tempers boiling over here! Don't tell these players there's nothing to play for! Nyom and Olsson come together on the touchline and the latter reacts to something said by the West Brom right-back. It's all a bit heated at the moment, but a long talking to from referee Mike Dean is enough to settle things down for now.
35'
Looking for an immediate response, Swansea are forward and have a free-kick in a threatening area. Sigurdsson stands over it and lines up a strike from just over 25 yards, but his effort is straight into the waiting arms of Foster, who quickly starts the Baggies forward on the break.
33'
J. Evans has scored a goal for West Bromwich Albion! Assist by D. Fletcher.
There was nothing special behind that delivery from Fletcher, who provides the assist. It was just poor coverage from Swansea all-around.
WEST BROM DRAW FIRST BLOOD! It's far too easy for the Baggies as Evans is left unmarked to head home Fletcher's corner at the far post, with Nordfeldt caught in no-man's land! That's one that will have Clement pulling his hair out.
31'
SOMEHOW NOT IN! Swansea do well to keep play alive following a corner, as it's delivered again into the centre. Fer arrives first and takes a stab at it, but it deflects wide! Pressure stays on and Llorente gets on the end of a cross at the far post, but he too bounces an effort wide!
29'
West Brom have completely suffocated Swansea's creative types in midfield. Sigurdsson, Fer and Carroll have had very little time and space on the ball to carve out a chance, far too often having to retreat back to the defence. This negative approach in possession has seen Swansea struggle mightily to generate any sort of threat in attack.
27'
STAT: Salomon Rondon has scored four goals in his last two appearances against Swansea, including a hat-trick of headers in the reverse fixture back in December at the Hawthorns.
25'
Rondon continues to look up for the occasion. He's proved to be a consistent target up top and is doing a good job of holding the ball up for the Baggies as he continues to become more and more involved, showing good determination to add to his tally before the close of the season.
23'
Swansea respond with a decent attack of their own, Sigurdsson again at the heart of things, but they are forced to settle for a corner, which leads to nothing in the end. Momentum is slowly starting to swing in favour of the hosts, but their end product has been anything but inspiring.
21'
FIRST REAL CHANCE! Rondon gets on the end of a well-worked move and finds himself through on goal against Nordfeldt, on the end of Fletcher's throughball, but the keeper reacts stunningly to deny West Brom's leading scorer from close range!
19'
The supporters remain in high spirits despite the sluggish pace of the match unfolding in front of them, and they have every reason to be. The sun is shining on this beautiful May weekend, the Swans are set to compete in the Premier League next season and so to are the Baggies.
17'
STAT: West Brom have a dismal final day record in the Premier League, winning just once in 10 seasons (D6 L3), with that one victory coming in May 2005, when they started the day in 20th but managed to survive and finish in 17th.
15'
It has been fairly even through the first quarter of an hour. Both sides have poked and prodded attack, yet to really carve out a chance, but there are signs that this match can spring to life at any moment. A goal would certainly help that cause.
13'
Flashed over! Clever one-two between Sigurdsson and Fer as Swansea orchestrate their first real attack of the match. The return pass is prompt and it sends the Dutch midfielder through in the right channel, but his effort from a tight angle is lashed wildly over the bar.
11'
Decent build-up play from the Baggies as Rondon gets on the end of a cross in from the left, but can't generate the necessary power behind it to really test Nordfeldt. The keeper is deputising today for Fabianski, who had not missed a minute of action for the Swans this season before today, so it's curious as to why he's resting here today.
9'
Plenty of patient, periphery possession for both sides as the tempo struggles to pick up any sort of rhythm in these opening stages. Neither team is keen to really push the pace, at least not yet, and it's made for a rather sluggish start.
7'
STAT: Since Paul Clement’s arrival at Swansea City the Swans have won 26 points in the Premier League, which is more than 12 other clubs in this period.
5'
Given the circumstances, this could prove to be a very entertaining match as both sides can attack with reckless abandon, going home happy tonight regardless of win, lose or draw. Of course it could go the other way and prove to be dull and dreary on the eyes, with motivation possibly in short supply.
3'
Some block, that! West Brom negotiate the first encouraging attack of the afternoon as they work down the right flank. The initial ball in is dealt with by Swansea, but only partially as it comes to Chris Brunt to smack it on the volley, but Kyle Naughton throws himself in front of it fearlessly to block it aside!
1'
AND THEY'RE UNDERWAY! Kick-off is met by a passionate cheer from this sold-out crown as Swansea get us started here at the Liberty Stadium.
Hello and welcome to live coverage from the Liberty Stadium as Swansea City host West Brom as the 2016/17 Premier League season winds to a close.
This is very much a celebratory day at the Liberty Stadium as wins from three of their last four helped Swansea fend off the drop as they live to fight another season in the Premier League.
There's no overstating just how joyous a finale this is to be for Swansea, on what could have been a very different afternoon had they not strung together recent results. Paul Clement has done an excellent job with this side, as they somehow have managed survival with a game to spare, exceeding even the wildest expectations of the most optimistic of supporters.
West Brom have been anything but inspiring since the start of April. The Baggies reached the 40-point mark needed for survival and quickly tailed off, losing six of their last seven and scoring just three goals across their last eight, having not won a league fixture since March 18.
Remarkably, the Baggies are still positioned in the top half of the Premier League, speaking to the congested nature of the middle of the table. Outside of the top seven, no one has been able to separate themselves from the pack, with Tony Pulis’ side able to finish as high as eighth and as low as 11th.
Paul Clement makes two changes from the win at Sunderland, including one in goal as Kristoffer Nordfeldt replaces Lukasz Fabianski. Ki Sung-Yeung also drops to the bench to make room for Leroy Fer.
Tony Pulis makes just one change from the defeat at Manchester City, with James Morrison replacing Nacer Chadli on the right.
A free Jonny Evans header has the Baggies leading at the interval at a celebratory Liberty Stadium as they search for their first victory since the middle of March, up against a Swansea side who have struggled a bit for motivation.
Jordan Ayew and Fernando Llorente complete a remarkable turnaround for Swansea, a microcosm of their 2016/17 Premier League season, as they defy the odds, coming from behind to secure three points, much like they did in securing their top-flight status for next season!
Swansea end the season five games unbeaten, winning four, while West Brom close out the campaign winless in nine, losing seven, though Tony Pulis does finish in the top half of the table for the first time as a Premier League manager.
That's all for our live coverage from the Liberty Stadium, and for the 2016/17 Premier League season. Thank you all for coming along for the ride!
